APNA's Chronic Disease Management and Healthy Ageing (CDMHA) program provides an evidence-based education framework to support nurses in primary health care1 to increase their knowledge, skills, and confidence in this area.
Duration: Six months
Time commitment:
Compulsory: 6 x 2-hour virtual workshops (worth up to 10 CPD hours)Recommended: Community of Practice where resources will be sharedLocation: The workshop and all other activities take place online.
Cost: $220.00 for both APNA members and non-members. *As this program is subsidised by the Australian Government Department of Health and Aged Care, an APNA member discount cannot be applied.
Who can take part?
The program is for Registered Nurses, Enrolled Nurses, Midwives and Nurse Practitioners with experience in chronic disease management and aged care who are interested in expanding their knowledge of cardiovascular health, diabetes, mental health, cancer and injury prevention.
Tailored for nurses seeking to advance their skills in the focus areas of cardiovascular health, cancer control, injury prevention and control, mental health and diabetes mellitus. This course presents comprehensive strategies and is aimed at enhancing existing practices and expanding professional expertise. If chronic disease management is not currently in your existing practice, please view APNA Principles of Chronic Disease Management which is for nurses to develop and apply skills in chronic disease management, developing foundational knowledge and building practical strategies.

Workshop
Nurses participating in the program will attend one 2-hour workshop per month for six months. These interactive workshops provide an evidence-based, best-practice framework for managing chronic disease and promoting healthy ageing with a focus on the following key health topics:
Cardiovascular HealthCancer controlInjury prevention and controlMental HealthDiabetes MellitusAt the commencement of the program, nurses will complete a self-assessment (online survey) to assess their current knowledge and identify any areas for improvement.
Community of Practice
A private messaging platform where program participants can discuss, engage and consult with their peers. APNA provides tips and resources to the group, encouraging and facilitating discussion.
Follow-up support
This will include one complimentary call for non-members and ongoing support to members through the APNA Nurse Support Line – a service to assist nurses with queries. APNA may also make contact to check how nurses have been implementing learnings from this program.

APNA First Nations CDMHA Bursary
The CDMHA First Nations Nurse Program Fee Bursary is intended to support the participation of Aboriginal and Torres Strait Islander nurses (Enrolled Nurses, Registered Nurses and Nurse Practitioners) in the Nursing in Primary Health Care (NiPHC) Chronic Disease Management and Healthy Ageing (CDMHA) program. A limited number of bursary places are available in each CDMHA program intake.
