Effect of Community Chronic Disease Management on Secondary Prevention of Hypertension with Stroke

Objective To analyze the secondary prevention of community chronic disease management programs in patients with hypertension and stroke. Methods From September 2015 to September 2016, 70 patients with hypertension and stroke in this community hospital served as the control group, and started the community routine management mode; 70 cases of hypertension were taken from October 2016 to October 2017. The patients with stroke were used as experimental group to carry out chronic disease management in the community, and the treatment of hypertension and the recurrence of stroke were recorded. Results After the management of chronic diseases in the community, the diastolic blood pressure and systolic blood pressure of the experimental group were lower than those of the control group, and there were 3 cases in the experimental group, and the stroke was recurred, while the control group had 15 cases(P<0.05). Conclusion The development of a community chronic disease management program in patients with hypertension and stroke can improve the blood pressure level of the body and prevent the recurrence of stroke, which is worth promoting.
